How to register and login
Create your member account on the LIBERTY Dental client portal to view claims and member information. You register with your name as it appears on file and your member number, which ends in -01 for the primary account holder.

To find an in network dentist online, open the Provider Directory on your state member site, for example libertydentalplan.com/FLMedicaid, and search by location.
Plan
LIBERTY Dental Plan covers Medicaid dental services that vary by state and age group. In Florida, for example, the program covers children from newborns through age 20, pregnant women age 21 and older, and adults age 21 and older.
Your exact covered services, limits and copays are listed in your state Enrollee or Member Handbook on the member site.
How to activate
Once you are enrolled in Medicaid and assigned to LIBERTY for dental, activate your benefits by registering on the member portal and selecting a Dental Home, which is your primary dental provider.
You will receive an ID card from LIBERTY in addition to your state Medicaid card, so bring both cards to every dental appointment.
Manage your benefits
Use the member portal to check claims, review covered services and update your Dental Home. Member Services can help you find the closest participating dentist and assist with appointments.
