How to register and login
Create a Colorado PEAK account at Colorado.gov/PEAK to apply for Health First Colorado or to manage an existing case. You can also apply by phone at 1-800-221-3943.
If you applied by mail, in person or by phone, you can still set up a PEAK account afterward to check your application status, view benefits and print a replacement card.
For password resets, error messages or navigation problems, contact PEAK Technical Support at 1-800-250-7741 or use the 24/7 chat on the PEAK site.
Plan
Health First Colorado offers physical health plans that include vision and prescriptions, plus behavioral health coverage. Dental benefits are provided through DentaQuest.
Children and pregnant women who earn slightly too much for Medicaid may qualify instead for Child Health Plan Plus, known as CHP+.
How to activate
Activate your coverage by completing your PEAK account and downloading the free Health First Colorado app to keep your information current and find a doctor or dentist.
Tell the program if you become pregnant using PEAK, the app, or by calling 1-800-221-3943, so you receive the full range of prenatal benefits.
Manage your benefits
Use PEAK to report changes, complete your yearly redetermination and upload verification documents. Keep your phone number, email and mailing address up to date so you do not miss renewal notices.
For non emergency rides to appointments, schedule transport online with the plan or by phone, and call the Nurse Advice Line at 1-800-283-3221 when you are unsure about a symptom.
