If you provided your email address an invitation has already been sent to you.<\/li>\n<li>Follow the email instructions to enroll. You will need:\n<ul>\n<li>First and last name<\/li>\n<li>Date of birth<\/li>\n<li>Zip Code<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3><span style=\"color: #800080;\">Option 2: Self Enrollment<\/span><\/h3>\n<div class=\"list-content\">\n<ol>\n<li>Click \u201cEnroll in myMedStar\u201d at the top of this page.<\/li>\n<li>Follow the instructions to enroll.
