This includes a range of services from checkups to surgeries to home health care. It also includes prescriptions and medical equipment. Apply online now.<\/p>\n<h2><span style=\"color: #800000;\">Why Register<\/span><\/h2>\n<p>Access is open to anyone. However, this level requires you get a My HealtheVet account. You do this by\u00a0<a href=\"https:\/\/www.myhealth.va.gov\/why-register\">registering<\/a>. Anyone can register; you do not have to be a Veteran. When you register, it lets you personalize your health information. You may also use the journals and other tools to track your health measures. You can only see this information when you are signed in, so check it out and\u00a0<a href=\"https:\/\/www.myhealth.va.gov\/user-registration\">register today<\/a>. As a registered user, you may use My HealtheVet to:<\/p>\n<ul>\n<li>Add information to a personal health journal about over-the-counter medications, allergies, military health history, medical events, tests and allergies<\/li>\n<li>Record and track personal information such as contact information, emergency contacts, health care doctors and providers, and health insurance information<\/li>\n<li>Record and track personal health measurements (blood pressure, blood sugar, cholesterol, heart rate, body temperature, weight, pain level, etc.) in Vitals &amp; Readings<\/li>\n<li>Print a wallet ID card with the personal information entered into the personal health record<\/li>\n<li>Use\u00a0<strong>VA Blue Button<\/strong>\u00a0(Download My Data) to view, save download and\/or print your self-entered information; then share this with your caregiver, non-VA provider or others you trust<\/li>\n<\/ul>\n<h2><span style=\"color: #800000;\">Veterans<\/span><\/h2>\n<p>If you are a Veteran enrolled in the VA Health Care System, as a registered user you may use My HealtheVet to refill your VA prescriptions online. Just make sure when you register you:<\/p>\n<ul>\n<li>Enter your full name (including middle name) on the form<\/li>\n<li>Enter your gender, Social Security Number and birth date<\/li>\n<li>Check\u00a0<strong>VA Patient<\/strong>\u00a0as your Relationship to the VA<\/li>\n<\/ul>\n<p>After you select\u00a0<strong>VA Patient<\/strong>, My HealtheVet matches the following information you entered on the Registration Form against the VA&#8217;s Master Veteran Index database:<\/p>\n<ul>\n<li>Full name (first, middle and last name)<\/li>\n<li>Social Security Number<\/li>\n<li>Gender<\/li>\n<li>Date of Birth<\/li>\n<\/ul>\n<p>This process verifies your identity as a VA Patient. Gather your information<\/h3>\n<ol>\n<li class=\"process-step list-two\">Here\u2019s what you\u2019ll need to apply:\n<ul>\n<li><strong>Social Security numbers<\/strong>\u00a0for you, your spouse, and your qualified dependents.<\/li>\n<li><strong>Your military discharge information<\/strong>\u00a0If you sign in to apply, we may be able to fill in this information for you. If you don\u2019t sign in to apply, we\u2019ll ask you to upload a copy of your DD214 or other separation documents.<\/li>\n<li><strong>Insurance cards<\/strong>\u00a0for all health insurance companies that cover you. This includes any coverage that you get through a spouse or significant other. This also includes Medicare, private insurance, or insurance from your employer.<\/li>\n<\/ul>\n<p>We\u2019ll also ask you for this optional information:<\/p>\n<ul>\n<li><strong>Last year\u2019s gross household income<\/strong>\u00a0for you, your spouse, and your dependents. This includes income from a job and any other sources. Gross household income is your income before taxes and any other deductions.<\/li>\n<li><strong>Your deductible expenses for last year<\/strong>\u00a0These include certain health care and education costs. These expenses will lower the amount of money we count as your income.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<div>\n<h3 class=\"vads-u-font-size--h4\">3. All the steps listed below must be followed.<\/p>\n<ul>\n<li>Sign in to My HealtheVet using your Premium DS Logon or ID.me* account<\/li>\n<li>You may be asked to accept the My HealtheVet Terms and Conditions<\/li>\n<li>Select the &#8216;Upgrade &#8216; button on the home page next to your name<\/li>\n<li>Check the certification box to verify you&#8217;re the owner of the account and select &#8216;Continue&#8217;<\/li>\n<li>After selecting &#8216;Continue,&#8217; your account will upgrade to Premium<\/li>\n<\/ul>\n<p><strong>*ID.me:<\/strong>\u00a0This is a trusted VA partner that meets the U.S. requirements for online identity proofing and authentication. You can register using your email address, or with a\u00a0<strong>Facebook, Google,<\/strong>\u00a0or\u00a0<strong>LinkedIn<\/strong>\u00a0account. Once you have an ID.me account, you can easily upgrade your My HealtheVet account to Premium.<\/p>\n<h3><strong>Get a Premium account in person<\/strong><\/h3>\n<p>You can get a My HealtheVet Premium account in person at a VA health care facility where you&#8217;re registered as a patient. You&#8217;ll need to bring a government-issued photo ID.
