You can\u2019t always make it to your doctor\u2019s office. With Email and Video Visits, you don\u2019t have to for certain symptoms. Receive care from your trusted physician by answering a few questions through your mobile device or have a video call to discuss any issues face-to-face. You can also send a secure message to ask questions, request prescription refills, and view your test and lab results, too, without a complicated game of phone tag.<\/p>\n

    Create an Account from an Invitation<\/span><\/h2>\n

    If you have received an invitation and are creating your account from a mobile device, go\u00a0here.<\/em><\/p>\n

    If you are looking to create your FollowMyHealth\u00a0account from an invitation, please follow the steps below:<\/p>\n

    Click on the registration link from your email. You will then see the following screens with the below options.<\/p>\n

    Step 1:<\/strong>\u00a0Select the \u201cSign Up and Connect\u2019 option.<\/p>\n

    Step 2:<\/strong>\u00a0The username and password screen. By default, the email address where your invitation was sent will be in the username field. It is recommended that you use your email address as your username. If you share an email address, or you want to use a different username, you can edit this field.<\/p>\n

    Step 3:<\/strong>\u00a0Accept the Terms of Service<\/p>\n

      \n
    • Read through the Terms and select the \u201cI Accept\u201d button on the bottom of the window.<\/li>\n
    • Please note, by hitting \u201cI Decline\u201d, the account creation process will not complete.<\/li>\n<\/ul>\n

      Step 4:<\/strong>\u00a0Enter the Invite Code<\/p>\n

        \n
      • This is the invite code entry. It can be found in the invitation you received from your medical organization.<\/li>\n
      • Input the Invite Code in the box and select \u201cNext\u201d<\/li>\n<\/ul>\n

        Step 5:<\/strong>\u00a0Request for Access<\/p>\n

          \n
        • Read through the Request for Access and select the \u201cI Accept\u201d button on the bottom of the window.<\/li>\n
        • By hitting \u201cI Decline\u201d, the account creation process will not complete. You will not be able to see your medical information.<\/li>\n<\/ul>\n

          Step 6:<\/strong>\u00a0Upload of the Health Record to the Platform<\/p>\n

          • A set of animations will display as aspects of your Health Record is being placed in the platform.<\/li>\n<\/ul>\n
