Opens in new window\">www.vsa.maryland.gov.<\/a><\/p>\n<h2 class=\"subhead1\"><span style=\"color: #800000;\">Who Can Request a Birth Certificate<\/span><\/h2>\n<p>You may obtain a\u00a0Maryland Birth Certificate\u00a0if you are:<\/p>\n<ul>\n<li>Person requesting his or her birth certificate<\/li>\n<li>Parent(s) whose name is on the birth certificate<\/li>\n<li>Guardian with court certified documents showing legal permanent custody<\/li>\n<li>Person with a notarized letter of authorization from the requestor<\/li>\n<\/ul>\n<h2 class=\"subhead1\"><span style=\"color: #800000;\">Identification Required<\/span><\/h2>\n<p>For same day service, applicants are required to present a valid, government-issued photo identification that includes an expiration date. i.e. (Pass port, drivers or non-driver\u2019s license). Applicants who are unable to supply this type of identification must present two different items from the following list. At least one of these documents must contain the applicant\u2019s current mailing address. Applicants who cannot provide valid photo identification will not receive the birth certificate the same day.
