The survey must be completed before coming to campus or arriving at class.<\/p>\n<ul id=\"block-fa5e395d-6272-4f57-873d-0c143ec4a97a\">\n<li><strong>Take the Symptom Monitoring Survey<\/strong><\/li>\n<li><strong>Check the COVID-19 Clearance Portal<\/strong><\/li>\n<\/ul>\n<p>The brief form can be filled out on a smartphone, tablet or computer (and can now be accessed through the UCLA Mobile app, available on the\u00a0App Store\u00a0and the\u00a0Google Play Store).<\/p>\n<p>Students, faculty and staff can also obtain their\u00a0<strong>Events &amp; Venues Clearance Certificate<\/strong>\u00a0through the survey on the UCLA Mobile app. This digital certificate will allow you to enter any campus location covered by UCLA\u2019s Protocol for Restaurants and Other Public Venues.\u00a0See protocol details and campus locations. This certificate is currently only available through the mobile app.<\/p>\n<p><img decoding=\"async\" class=\"aligncenter\" src=\"https:\/\/compmed.ucla.edu\/sites\/default\/files\/styles\/default_max\/public\/page\/img\/main\/Compmed_bannerweb_Dailysymptomx2_0.png?itok=QwMxzvjy\" alt=\"Covid-19 Symptom Screener\" width=\"663\" height=\"442\" \/><\/p>\n<p><strong>Fully remote employees:\u00a0<\/strong>Employees working fully remotely must update their status one time within the system to indicate to indicate they are fully remote as soon as possible.<\/p>\n<p><strong>Hybrid employees<\/strong>: Employees working a hybrid schedule should complete the survey only on the days when they will be working in person.<\/p>\n<p>I<strong>nvited guests and third-party contractors:\u00a0<\/strong>All guest lecturers, vendors, third-party contract workers, external parties participating in trainings or programs, and other invited guests defined as \u201ccovered non-affiliates\u201d in the\u00a0UC COVID-19 Vaccination Program Policy\u00a0must complete the survey before coming to campus.<\/p>\n<h2><span style=\"color: #800000;\">Verifying your campus clearance<\/span><\/h2>\n<p>After completing the survey, students, faculty and staff can view their symptom monitoring clearance status on the\u00a0COVID-19 Clearance Portal\u00a0and can directly access their COVID-19 Clearance Certificate. Individuals may be asked to present their certificate or to log into the portal as proof of completion when necessary. Both can be accessed through the web or the UCLA Mobile app.<\/p>\n<p>They can also directly access their\u00a0<strong>Events &amp; Venues Clearance Certificate<\/strong>, which will allow them convenient entry to\u00a0certain facilities on campus\u00a0\u2014 including most restaurants and cafes, fitness centers, theaters, performance spaces, museums and galleries.
