Ordering your quarterly supply online is easy! Visit SeniorCarePlus.NationsBenefits.com to set up your account by registering on the NationsBenefits MyBenefits portal. Once you are set up, you can view all products and place online orders.

2. Enter the 16 digit card number and expiration date of your Benefits Mastercard® Prepaid Card
3. Enter your Date of Birth and Last Name provided to your health plan during enrollment
4. Finally Click on the Activate Card Button

How to order
There are three easy ways to place your order: by mail, by phone or online. Whatever way you choose to place your order, be sure to have your member ID number available to access your benefit.

Fill out the attached mail order form and drop it in the mail to:

Be sure to give the order seven days to reach us. The order will be processed in the quarter that it is received, unless otherwise specified. A new mail order form is sent with each of your quarterly shipments.
