To activate your HealthSun Grocery Card and balance check, login into healthsun.nationsbenefits.com or call 833-689-5169 (TTY: 711) and enter the 16 digit card number and expiration date of your Benefits Mastercard® Prepaid Card.

HealthSun Health Plans is an HMO plan with a Medicare Contract and a Medicaid contract with the State of Florida Agency for Health Care Administration. Enrollment in HealthSun Health Plans depends on contract renewal. Every year, Medicare evaluates plans based on a 5-star rating system. Star rating is for contract years 2018, 2019, 2020, 2021, 2022 and 2023.

The healthy foods grocery card is part of the special supplemental program for the chronically ill. Not all members qualify. Results from PRP treatment are not guaranteed and may vary by age and ailment. Please consult your doctor to learn more about the benefits, limitations and possible side effects.

The Healthy Meals Program requires participation in Care Management Program. Nutritional assessment, prior approval, and referral are required. Other restrictions apply. This benefit is part of special supplemental benefits. Not all members will qualify.

Eating healthier can be challenging, especially for individuals who lack transportation or the financial means to buy healthy groceries. We make accessing healthy foods simple and hassle-free for your members.

Healthy Groceries Card $0 copay

• A monthly allowance in the form of a debit card to buy a wide range of approved groceries for healthy food and produce, including
supplemental drinks.
$75 maximum monthly benefit amount for your groceries.
Members are eligible based on low income status qualifications for the dual eligible special needs plan (LIS levels 1, 2, 3, or 4). Unused amounts do not roll over to the next month or year.

Once you receive and activate your card, you can start using it online through the MyBenefits portal and the MyBenefits app and/or in person at a participating retail location near you. When at a retail location, simply swipe your card for payment.

1. Visit healthsun.nationsbenefits.com or call 833-689-5169 (TTY: 711)

2. Enter the 16 digit card number and expiration date of your Benefits Mastercard® Prepaid Card.

3. Your 12-digit member ID contains 2 letters and 10 numbers.

4. Enter your Date of Birth and Last Name provided to your health plan during enrollment.

5. Click on Activate Card Button to activate your card.

Joining HealthSun

To enroll with HealthSun Health Plans, you must have Medicare Part A and Medicare Part B and live in one of our covered service areas. View our Summary of Benefits for plans and service areas available to you.

Typically, beneficiaries cannot enroll in a Medicare Advantage Plan outside of the Open Enrollment Period. However, in certain situations a Special Election Period (SEP) may be used during the other months of the year so that beneficiaries may be able to join, switch, or drop a Medicare Advantage HMO plan or a Prescription Drug plan.
