How To Report Medicaid Fraud Anonymously | TTY: 1-800-377-4950

You can report Medicaid fraud anonymously โ€” every U.S. state (and the federal government) provides ways to do this without giving your name.

Hereโ€™s how ๐Ÿ‘‡

๐Ÿ›๏ธ 1. Federal (U.S. Department of Health & Human Services)

If youโ€™re unsure which state it happened in โ€” or want to go straight to the top level:

  • Office of Inspector General (OIG) โ€“ U.S. Dept. of Health & Human Services (HHS)
    ๐ŸŒ Website: https://oig.hhs.gov/fraud/report-fraud/
    โ˜Ž๏ธ Phone: 1-800-HHS-TIPS (1-800-447-8477)
    ๐Ÿ“  TTY: 1-800-377-4950
โœ‰๏ธ You can submit tips anonymously online โ€” you donโ€™t need to provide your name or contact info.

Use this if you suspect:

  • A provider billing Medicaid for services not provided,

  • Identity theft or falsified claims, or

  • Kickbacks or false documentation.

๐Ÿ™๏ธ 2. Report to Your Stateโ€™s Medicaid Fraud Control Unit (MFCU)

Each stateโ€™s Attorney General runs an MFCU.

You can find your stateโ€™s contact info here:

You can report:

  • Fraud by doctors, pharmacies, or home-health agencies

  • Fraud by beneficiaries (selling benefits, lying on applications, etc.)

Most states allow anonymous or confidential reports via:

  • Online forms

  • Fraud hotlines

  • Mail or fax
